Alcala Care Management Services is there to help families with the struggles we all face when caring for an older adult. Caring for a parent, aunt, uncle, grandparent, friend or neighbor as they age is not an easy task. Changes often happen quickly and we may be caught off guard by many of them.

What is Geriatric Care Management?
Care management is a guide for families experiencing changes due to the effects of various disease processes. These services are intended to support families through the decision-making process in order to achieve the best possible outcomes for that particular family.
A professional care manager works with the family in a variety of ways:
- Act as an advocate for the client to make sure their voice is heard throughout the process.
- Help arrange care following a hospital or skilled nursing discharge, including caregiver interviews to make sure they are a good fit for the client's needs and wants.
- Deliver care options including skilled nursing, assisted living, board & care, adult day care, home care and hospice.
- Investigate insurance coverage including Medicare and Long-Term Care Insurance.
- Educate family caregivers on a variety of disease processes including caregiving, stroke, cancer, Alzheimer's Disease and other types of dementia.
- Attend medical appointments to enable the efficient transfer of information between client, caregivers, family and doctors.
- Assist the client's caregivers to help ensure proper follow-through of physicians' orders.
- Work alongside trust officers, conservators and other fiduciaries to manage the client's health care needs.
- Work within the dynamics of the individual family to achieve the best possible outcome for the client.
